There were two times: one was in the Star Trek: The Original Series and Star Trek: The Remastered Original Series episode "The Menagerie". The other was in Star Trek II: The Wrath of Khan.
Recheck your facts the only time Spock committed mutiny was in "The Menagerie". Nothing Spock did in The Wrath of Khan could be considered mutiny all he did was act to save the ship. The only way anything he did could be considered mutiny is if Kirk had given him a direct order to stay on the bridge. Whether you look at him as the Captain or the first officer of the Enterprise either position gave him the authority to do what was necessary to save the ship.
I actually just meant that Spock took over as Captain of the Enterprise. True, it was not shown to be that big of a deal between the two of them, but after the events of "The Menagerie"...
