I would say that "death", as it is clinically defined, did occur to Spock in Star Trek II: The Wrath of Khan. At the end of the Star Trek: The Next Generation episode "Unification II", it seems clear that Spock is alive, although on Romulus. Scotty pretended to die in the Star Trek: The Original Series and Star Trek: The Remastered Original Series episode "I, Mudd". Scotty departed the Enterprise-D, in one of its shuttlecraft, in the Star Trek: The Next Generation episode "Relics"; therefore, whether or not Scotty has died is open to debate, as the event has never been depicted in canon, but it stands to reason that it certainly may have occurred ProfessorTrek 21:32, July 16, 2011 (UTC)

Real life: James Doohan died on July 20, 2005. Leonard Nimoy is alive. Zachary Quinto is alive. Whether Carl Steven, Vadia Potenza, Stephen Manley or Joe W. Davis remain alive, is unknown to this author ProfessorTrek 21:38, July 16, 2011 (UTC)
